Job Summary:
As an External Coordinator at Comfort Keepers, you’ll ensure continuity of care for clients across Hazleton, Pottsville, Selinsgrove, Cleona, and Bloomsburg. Collaborate with our Support Team to coordinate in-home services, manage client care resources, and connect with staff, families, community providers, and third-party payers.

Essential Functions:

  • Handle client inquiries, qualify leads, and schedule visits
  • Assess client and caregiver needs, ensuring safety and quality care
  • Coordinate shifts, resolve scheduling issues, and conduct regular visits
  • Facilitate caregiver and family training
  • Maintain compliance through documentation and care monitoring
  • Serve as a vital link between clients, providers, and the Client Services team
  • Participate in on-call rotations and management meetings
  • Complete administrative tasks and drive care improvements
Qualifications:

  • Proficiency in Microsoft Office and related software
  • Excellent communication, organizational, and problem-solving skills
  • Strong sales abilities and business acumen
