We are seeking a dependable and organized Home Care Coordination Specialist to join our Bethlehem-area team. This role is ideal for someone who can manage multiple tasks, communicate effectively, and help ensure smooth daily home care operations.
In this position, you will help coordinate caregiver schedules, support communication between families and staff, and keep client care running consistently and efficiently. You will be an important part of making sure clients receive timely and reliable support every day.
Responsibilities include:
- Coordinating and updating caregiver schedules based on client needs
- Communicating changes, updates, and instructions to caregivers and families
- Handling urgent schedule adjustments and last-minute changes
- Maintaining accurate and organized client records
- Supporting overall daily care coordination and workflow
